To access it, open the patient’s chart and either hover over the “O” icon or click Objective, then select Range of Motion from the dropdown menu. The screen will display a multi‑panel view showing the most recent ROM findings for each region. Select any panel—or use the tabs on the left—to review or update an existing entry.

You can modify findings using the sliders, spinner arrows, or by typing values directly. If the patient demonstrates normal motion, simply click the Normal button. To the right of this button, the system displays the current deficit compared to normal values. On the far right, a pain‑level slider allows you to document discomfort at levels 1, 2, or 3, corresponding to mild, moderate, or severe pain.

To enter new findings, choose any unused tab for the region you wish to document.

Subjective Complaints

Subjective Complaints are included in both SOAP and Narrative reports. When used in Narrative reports, any "Initial" report template will access the first subjective complaint findings entered for the patient. "Current" and "Final" reports will use the most current subjective complaint findings

History of Occurrence §/Medical, Family,& Social History

The History of Occurrence Window is a free-form page where you can type any patient history. It can be ‘forced’ into a SOAP note by checking the appropriate box on the Documents Window. The same goes for Medical, Family,& Social History tab on the History of Occurrence Window. This allows you to control when you want the information in your notes as you most likely would only use them in an initial note. There are other Windows that only generate in the SOAP when forced. They are all noted with the “§” symbol next to the name.

Medical History

Only generates in Narrative report templates.

Family History

Only generates in Narrative report templates.

Auto Accident

Only generates in Narrative report templates designated as for Auto Accident such as Auto Initial, etc.

Work Injury

Only generates in Narrative reports templates designated as Work Injury.

MyEMR also supports "Batch" printing multiple patient notes by date range, Account Type, or Category (see SOAP Note Batch Printing).

Printing Patient SOAP Notes (paper)

Once a SOAP notes are expressed and saved, the notes are stored in the Prior tab on the Documents Window. This is where you go to print patient SOAP notes.

Change Date on Express (New) Note

Once you Express a new note and are happy with the results, you can modify the date of the note in the upper left corner of the Documents Window. Look for the date field with today's date, and click the arrow to open a calendar to change the date.

Change Date on Saved (Prior) Note

Once you have saved a note, you can change the date on the Prior tab using the following steps:

Click the Prior tab on the Documents Window.

Right-click on the date of the note you wish to change.

Choose Properties.

Click the arrow to the right of the date on the Edit SOAP Note Details Window and use the calendar to set the appropriate date.

NOTE: It is not necessary to change the time as it is used as an index and does not appear in the printed note.

Parts of a Modality

There are four (4) parts to a modality. They are:

Mnemonic - The mnemonic is a code or an abbreviation used to identify the modality.

Listbox Description - The Listbox description is the name of the modality as you want it to appear in the list on the Plan Window.

Synonyms - The synonyms are part of the verbiage that generates in the note. You may create several iterations to describe the modality to allow the text engine to randomize the verbiage. However, you need at least one (1) synonym for the modality to appear in a note. Synonyms should be entered using lower case letters unless capitalization is required. Optionally, you may add a 'timed amount' to the synonym such as: 15 minutes of massage.

Goals - The goals describe the expected results from the modality. Like synonyms, you need to enter at least one (1) goal for the modality to generate in a note. However entering several iterations will allow the text engine to randomize the goals when generating the note. Goals should be entered as a sentence fragment without.

Modality Plan Templates

The factory plan templates are used to merge in the modality (synonym), region, and goals. While you can add or modify these templates, it usually is not necessary.

Add, Clone, Edit Modalities

As mentioned you can Add or Clone a modality. Additionally, you can Edit an existing Modality. Once you have completed all your changes, make sure you refresh the local verbiage. Close any open patients, then Go to Tolls > Refresh Local Verbiage and wait for it to complete.

Clone existing modality

The easiest way to add a new modality is to 'clone' and existing one. When you clone an existing modality, the newly created modality inherits all the verbiage from the original modality, meaning only slight changes to the verbiage are needed. For example, if you 'clone' Hot Packs to create Cold packs, you only need to change the synonyms by replacing the word Hot with Cold.

To clone an existing modality:

Go to Tools > Customize > Plan > Modalities.

Click on the Modality you wish to Clone.

Click the Clone Item... button.

Change the Mnemonic and Listbox Description for the new Modality.

Click OK.

Click on Synonyms...

Make any appropriate changes to the first iteration to reflect the new Modality.

Use the Random Iteration arrows to scroll through any other iterations and make the necessary changes.

Click OK.

Go to Tools > Refresh Local Verbiage or (Shift+F5) to distribute the changes.

Add New Modality

To add a new modality, you follow the same steps as you do to Clone an existing modality, with the following exceptions:

Do not click on an existing item.

Click the New Item... button.

Create a new Mnemonic and Listbox item.

Create new synonym(s)

Create new Goal(s)

Go to Tools > Refresh Local Verbiage or (Shift+F5) to distribute the changes.

Edit Modality

You can Edit the Listbox Description, Synonym(s) and/or Goal(s). You cannot change the Mnemonic. To Edit a modality:

Click on the Modality you wish to edit.

Click Edit Item...

Make the changes to the Listbox, synonym(s) or goal(s).

Go to Tools > Refresh Local Verbiage or (Shift+F5) to distribute the changes.

When customizing text, you should rarely modify the factory default modality & region templates. They are designed to accept any modality and any region, which gets merged into the template upon text generation. These templates are published if you need to restore your plan templates to the factory default.

Go to Tools > Customize > Plan > Modalities.

Click on the Templates button.

Use the right arrow in the Random Iteration # section to move to the next iteration.

If you changed that iteration, copy and paste the second Factory Template here.

Repeat until you have all the Factory Templates restored.

After customizing verbiage, go to Tools > Refresh Local Verbiage to copy the changes to the verbiage folder
