This prevents duplicate submissions or printouts.

However, you can still retrieve batches of claims that have already been submitted or printed. Each batch is timestamped with the exact date and time it was created—note that this may differ from the actual dates of service.

To locate a specific batch, you’ll need to estimate how many days ago it was created. This helps you define the correct Days Open range for retrieval.

Steps to Retrieve and Requeue Claims

Go to Tools > Insurance Manager or click the Insurance Manager icon on the left toolbar

Select the Claim Registry tab.

Choose the appropriate Form Type (e.g., DiskImage 0212 or CMS-1500 0212).

Click the Filter icon next to the Form Type.

In the Days Open fields, enter the date range (e.g.,
0
and
2
for claims created in the last two days).

Check the All Claims box above the Days Open fields.

Click Display to view the matching claims.

Right-click any claim and select Queue All to requeue the entire batch.

Click the SAVE icon to generate a
Claims0212
file or click the Printer icon to print paper claims.

What are Auto Checkout Macros?

Auto Checkout Macros allow you to "map" CPT Procedure Codes to your MyEMR for Windows® Modalities for speedy checkout in EMR Datacenter. Clicking the MyEMR button on top of the Transaction Window will populate the transaction with the patient's "mapped" Modalities.

Not all Modalities are mapped in the Factory database, and any 'procedures you may have added are not mapped.

Treatment Macros - are the Modalities in your Plan Window.

Available Procedures - are all the procedures in the Procedure Catalog.

Arrows - move the procedures back and forth between the Available Procedures and Procedures in Macro.

'Procedures in Macro' box.

Mapping Auto Checkout Macros

Auto Checkout Macros may be mapped in either EMR Datacenter or MyEMR for Windows®. To map the modalities, in either program:

In either program, click Tools > Catalogs > Procedures

Click the Auto Checkout Macros button.

Click the Modality in the Treatment Macros box to see if the procedures are mapped.

Scroll to click on the appropriate procedure in the Available Procedures box.

Click the arrow pointing to the right to assign the procedure to the Procedures in Macro box.

Repeat the previous steps for each macro you want to map.

When finished, close the Procedure Macros and Procedures Windows (there is no SAVE).

EMR Datacenter and MyEMR for Windows® share a factory set of Procedure or CPT codes as well as Accounting codes in Tools > Catalogs > Procedures. The procedures do not have their associated fees. So part of the initial setup is to:

Enter the fees for the procedures you will be using.

Add any new procedures that are not already in the Procedure Catalog.

Optionally 'map' the Auto Checkout Macros (see Auto Checkout Macros).

Entering Fees to Existing Procedures

The factory Procedure Catalog already has most common procedures entered with the exception of the Fee/Unit Cost (4). To enter the fees:

In either EMR Datacenter or MyEMR® go to Tools > Catalogs Procedures.

Use the Catalog ID list (12) or Search button (1) to lookup the procedure to add a fee.

Enter the amount in the Fee/Unit Cost box (4).

[Optional] If you offer a Time of Service Reduction, enter the amount of the discount in the TOSR Discount box (7). Click here for more on TOSR Discount

[Optional] If the procedure always requires a modifier, enter the modifier in the Modifier box (9). For modifiers specific to a certain Insurance payer(s), you would enter them in the payer's Intelli$ense. (For more information on Intelli$ense, Click Here).

You are not able to Delete procedures. You can set them Inactive in the Status(5) box. Inactive procedures will notshow in the Search or Lookup feature.

Creating New Procedure Codes

In either EMR Datacenter or MyEMR® go to Tools > Catalogs Procedures.

Click the New button. (1)

Enter a Catalog ID (2). The Catalog ID can be any combination of letters and/or numbers

Enter the Description (3).

The Status (5) can either be Active or Inactive. Inactive procedures will not be included in the Search or Lookup.

Choose a Category from the Category (6) drop-down list. If you want to add a new category, click the Categories button (1) on top to add a new category.

[Optional] If you offer a Time of Service Reduction, enter the amount of the discount in the TOSR Discount box (7). Click here for more on TOSR Discount

Enter the CPT code in the CPT Code box (8) if applicable.

Either (default) - allow the procedure to be billed to a third party payer, insurance, or the patient.

Third Part - the procedure can only be billed to Insurance or other third party payer (e.g. a report of findings fee).

Patient - the procedure is only billable to the patient (e.g. supplements).

In the Post Insurance Window, go to Global or Linet Item Posting.

Find the claim you want to submit to a secondary and click on it (if Line Item Posting, click on one of the lines in the claim).

Click the Resubmit drop-down and choose the appropriate payer (see picture below)

Click the Floppy Disk to Save the changes

Close the Post Insurance Window.

Reopen the Post Insurance Window, the claim will now be assigned to the chosen payer and queued in the Insurance Manager

ALL three (3) printed designs of the ledger are now formatted to fit in either #9 or #10 Double Window envelope when folded using a Z-Fold (see video below). There is a 'dashed' fold-line under the patient name and address to guide where to fold the paper to line up in the windows.

There are three (3) different formats for printing the Ledger: Simple, Detail and Attorney Clean Bill.

Simple

The simple ledger report includes only service and payment categories for the current view of the ledger. It contains the dates, quantities, description, amounts and the summary.

If ALL transactions are not included in the view, the report will state: *Not all transactions were selected for display. The summary will not add up to the total account balance.

Detail

The detail report includes more detailed information such as the ID and CPT codes of the transactions. It also includes the payer ID if there is a third party payer as well as if the charges are filed on a claim and what is insurance responsibility and/or patient responsibility. The summary is broken down by payer, patient, and total amounts.

If ALL transactions are not included in the view, the report will state: *Not all transactions were selected for display. The summary will not add up to the total account balance.

Attorney Clean Bill

The Attorney Clean Bill includes dates, CPT codes, Modifiers, Qty, Descriptions, DX codes, and amounts. The summary breaks down the patient, payer and total amounts due.

If ALL transactions are not included in the view, the report will state: *Not all transactions were selected for display. The summary will not add up to the total account balance.

TOSR: Time of Service Reduction

Time of service reduction ("TOSR") is a method of discounting services that does not discriminate against any person or responsible party in a healthcare provider office.

If services are paid at the time of service, a discount is appropriate since there will not be the administrative costs of billing and collecting the services, or the losses incurred through insurance (or discount plan) denials that, although candidates for administrative appeal with merit, the administrative costs outweigh appealing most services that are denied.

Accordingly, if a patient pays at time of service, it is fair to offer them a reasonable discount, proportional to your administrative collection fees, plus an incentive for a patient to pay at time of service.

This is non-discriminatory, because if an insurance adjuster was at the office with the company checkbook as their insured party checked-out, you would offer them the same TOSR discount. (Don't count on this.)

EMR Datacenter supports TOSR through the Tools->Procedure Catalog. For each service or product entry, the TOSR amount is the reduction. For example, if you charge $45 for an adjustment, but fairly assess your administrative payroll and other costs to collect the $45 as $8, then it is fair to implement the $8 TOSR discount. Similarly, if hot packs are $20, but the patient is willing to pay at the time of service, a fair TOSR discount may be $6.

The rule is that your TOSR discount is reasonable and prudent. The TOSR discounting method is not intended for hardship or charity cases, but the norm of your healthcare transactions.

Once you have decided your TOSR discount amounts and set them in Tools->Catalogs->Procedures, the TOSR discounts are ready to be implemented. On the New Transaction window, if the front desk person asks if the patient will be paying in full today, their "time of service discount" applies, so the TOSR button on the New Transaction window is pressed which applies the *TOSR discount to each transaction. (If a patient is able to pay some but not services at the time of service, the *TOSR must be deleted for transactions that will be billed – this is the "spirit" and also the legal argument of the TOSR discount.)

In real world situations, where insurance assignment is being accepted, the insurance companies will never receive the TOSR discount, because the insurance responsibility is assigned and billed. But if a patient has an insurance policy and you are not accepting assignment on the insurance, you do not have the administrative costs of billing so the patient should be able to qualify for the discount. You should seek legal advice in your state regarding the patient receiving a discount and their ability to receive proportional reimbursement when filing their own insurance.

Prepaid Wellness Care Plans and Group Discount Plans

Prepayment escrow plans are great for office, with emphasis that the legal agreement be construed such that the healthcare provider is not practicing insurance without a license.

Once again, since there is not the administrative cost of collecting the payment, this substantiates a reduced amount for the term of the escrow pre-payment, pursuant to a well constructed legal agreement. Before constructing any pre-payment escrow plan, you should check with a legal advisor who specializes in Healthcare Law in your state, or with your state's chiropractic association.

Group discount plans aren't themselves insurance companies, but rather are constructed to offer discounts for chiropractic, dental and other heathcare services. These patients are essentially cash patients who qualify for a discount negotiated by the group discount plan. These discounts should be entered as simple patient credit memos, or new patient credit memo entries can be created so that practice statistics can total the amount for each group discount plan.

Choosing the transaction

The first step for any of the above, is to find the transaction in the Ledger:

Open the patient chart.

Click the Ledger icon on the left site of the Front Desk.

Click Display for ALL transactions, or make the appropriate selections in the Find Transactions box, then click Display.

Find the transaction you want to work with.

Right-click the transaction and choose Edit Receipt.

Now that you have the transaction open, you may edit it or delete it.

Deleting the entire transaction

To delete the entire transaction, click the Clear Receipt button just below the transaction grid on the right.

Click Save (floppy disk) to "Save" the blank transaction.

Editing a receipt

Once you have the receipt open, you may make changes to the provider, dates, amounts, as well as where the transaction will be billed.

You may also add or delete line items.

Adding new line items

To add a line item while editing a receipt, click on the next line and enter the Catalog ID of the procedure you wish to add. Click the floppy disk to Save

Inserting a new line item

You may also 'insert' a line item between two (2) existing line items while editing a receipt:

Click on the line item below where you want to insert a new line.

Hold down your Ctrl key and press the Insert (INS) key on the keyboard (Ctrl+INS).

A new blank line item will be added above the line you selected.

Enter the Catalog ID for the procedure.

Click the Floppy Disk to Save.

Deleting a single line item

Instead of 'clearing' the receipt, you may choose to only delete certain line items on the receipt. To delete a single line item while editing a receipt:

Click on the line item you wish to delete.

Hold down your Ctr key and press the Delete (Del) key (Ctrl+Del).

Repeat as necessary.

Click the Floppy Disk to Save

In fact, they are both created at the same time. Instead of printing the claims to paper, you Save the claims to a file that is downloaded to your local desktop then uploaded to your clearinghouse.

Here are the the steps to filing claims electronically:

Before you create claims:

Make sure you are signed up with a clearinghouse that accepts print image files and everything is ready foryou to submit claims to them.

Go to Tools > Catalogs > Insurance Payers. For each payer you will bill electronically, set the Default Form Type to Disk Image 02/12. Additionally, if your clearinghouse has provided your with payer IDs for your payers, enter them in the Payer ID field.

For each existing patient you want to bill electronically, go to their Insurance Policies Window and on the Billing tab, make sure the Form Type is set to 'Disk Image 02/12' (future patient's form type will be set according to the Default Form Type for their payer).

Create claims as normal:

Go to Tools > Insurance Manager.

Check the Pre-Billing Scrubber tab for patient missing information and correct as needed (optional).

Click the Claim Registry Tab.

By default claims are created for transactions through the previous day. You may change that date if you choose.

Click the Create New Claims icon to create all new claims.

Click Yes to create the new claims.

Create Disk Image 02/12 File:

Choose Disk Image 02/12 from the Type drop down box.

You will see how many claims are selected and queued.

Click the floppy disk icon to create the disk image file.

Click Yes to create a new disk Image file. If you click No, the claims will be added to the existing file.

The queued claims will be save to a file named Claims0212.txt.

Download the Claims0212.txt file to the desktop of you local computer:

Keep EMR Datacenter open.

Locate the the MyEMR® SaaS Cloud Connector on your computer.

You will upload the Claims0212.txt file to your clearinghouse from your Windows Desktop.

How can I organize Imagecenter files in folders?

All files uploaded to the Imagecenter are uploaded to the Root Folder unless you are in the Patient or X-RAY Windows.

In preferences, you can tell EMR Datacenter to only put one (1) date of service on each claim. To do this, go to:

The report looks for any insurance transactions, not on an insurance claim, that do not have a corresponding SOAP note in MyEMR for Windows.

The report lists the patients in alphabetical order and lists the date(s) that are missing notes.

You run the report from:

Tools > Reports > Missing Progress Notes

The Missing Progress Notes report will:
